Full Job Description

We have an exciting opportunity for a Qualified Social Worker to join our forward-thinking company someone who is focused on arranging safe and timely discharges for patients either into their own home, Interim respite, or residential care., The charity arm of our business arranges and finances the safe discharge, respite and lifetime care of older homeowners back into their own home or a care home of their choice. We urgently seek an experienced Social Worker to join our passionate team in its mission to provide older people with the care they need. We seek applicants who recognise that neither NHS nor Local Authorities has or will have the funds to do this in the timely way required.

You will be working in the county of Monmouthshire on a full-time basis. This is a new post, and you will be working with our team at our offices in Pontypool and visiting hospital settings in the surrounding areas. It would be beneficial if you have worked within this area previously and already have positive working relationships with the discharge teams and the local authority.

Responsibility comprises the following:

  • Responsible for Discharge of patients with suitable services in place to meet their needs

  • Manage your own caseload and co-ordinate and monitor effectively and identify any gaps in services.

  • To maintain a range of resource information

  • Develop and maintain links with other multi-disciplinary teams within the area.

  • To manage statutory requirements under the Social Services and Well-being (Wales) Act 2014, Regulation and Inspection of Social Care Wales (Act) 2016, Mental Capacity Act 2005 and Deprivation of Liberty Safeguards, Equality Act 2010 and Human Rights Act 1998.

  • Safeguarding & Incapacity Assessments


  • This will be a fast-paced environment and your flexibility will be paramount in ensuring patients can be discharged on time and with the correct services in place.

    Applicants must be able to adhere to strict deadlines and be confident in their assessment skills taking account of a range of factors, including identifying and balancing need, risk, and rights and putting services in place sometimes with short notice.

  • Post-qualifying experience within a relevant team

  • Registered with Social Care Wales

  • A Degree in Social Work

  • CPD requirements are up to date

  • As the role will involve working with some vulnerable people, employment will be subject to appropriate pre-employment checks including referencing and an enhanced Disclosure and Barring check.

  • A driver with access to own car as travel is essential

  • Able to reside and work in the UK.

    · £40,000 OTE plus a monthly car allowance payment
