Full Job Description

Duties and Responsibilities: Professional Act in accordance with the Code: Professional standards of practice and behaviour for nurses, midwives, and nursing associates, and fulfil all registration requirements. To be familiar with and adhere to the Trust Code and Behaviour and Uniform and dress code policy. Respects the privacy, dignity, needs, beliefs, choices and preferences of patients and carers. Obtain informed consent for the identified care to be undertaken, ensuring the patient/client has a good understanding and knowledge of the decision-making process and is provided with accurate and appropriate information. Maintain clear and accurate patient records using both digital and paper-based records. Recognise and effectively respond to emergency situations requesting support from the registered nurse in a timely and appropriate manner. Work in accordance with the Health and Safety at Work Act 1974 and local policy and practices, including safeguarding and raising concerns. Use a
wide range of communication methods, including verbal, non-verbal and written, to interact with a variety of individuals including patients, carers, and members of the multi-professional team in a manner which is safe, effective, compassionate, and respectful. Recognise and consistently work within boundaries of the registered nurse role and their acquired skills and knowledge whilst developing competencies to progress their education and training for example skills in leadership and medicines management Work in an agile manner, able to work unsocial hours. Willingness to work in different locations within the Trust when required. Clinical Provide a high standard of safe individualised, holistic, and evidence-based patient focused advanced care in consultation with the patient, relatives, and the multidisciplinary team as per the plan of care. Demonstrate a working knowledge of the Care Programme Approach, Care Act, Mental Health Act and Mental Capacity Act Knowledge of current national
and local legislation and policy that impacts on mental health services. Minimise the Trusts environmental impact by recycling wherever possible. Act in a way that encourages and support equality and diversity that reflects the Trust values. Contribute to integrated care by working as part of the multidisciplinary team. Accurately document and report to other members of the nursing team and multidisciplinary team information regarding the care given to patients, communicating changes as they occur. To carry out mental health assessments on clients with other team members. Mental health nurses to administer and/or monitor medication as prescribed in keeping with policies and NMC requirements Managerial To be aware of the individual needs of other team members, giving support and guidance as necessary. To participate in orientation and induction of new staff and students on placement. To participate in appraisal and training of untrained staff and support the nursing team.
To act as a reflective practitioner, questioning clinical practice. Provide support and guidance to other staff as appropriate e.g support clinical and non-clinical staff, students. Participate in staff engagement meetings/activities and contribute positively to discussions regarding patients safety and care. Participate in data collection to support audit and contribute to quality improvement initiatives. To liaise with relatives, community mental health team staff, GPs, other hospitals, and local agencies to facilitate the smooth progression of care and aftercare of patients. To monitor and report serious untoward incidents/near misses so that appropriate remedial action can be taken. To liaise with relevant departments to ensure that supplies are available and delivered Education and Training Adhere to Trust and community infection control policies and guidelines relating to exposure to blood and body fluids and support. To be conversant with the current
curriculum for student nurses and contribute to the development of students, untrained staff and staff undertaking professional development.
